What people think it means: violently pulling hair off your head.
What it really means: to tear your hair out over something means to worry a lot about it. It is associated with anxiety, grief, anger and frustration. The phrase dates back to 1000 AD, when it had a more literal meaning!
And did you know, in the literal sense, the act of tearing one's hair out is the result of a compulsive disorder known as "trichtillomania".
